School Health Packet
Students in Kindergarten will need to meet the following requirements, mandated by the State of New Jersey before being allowed to start Kindergarten in September. All forms in the School Health Packet & documentation below should be submitted by August 15th. Please be assured that any information you provide will be kept confidential.
1. Immunization Record
2. Physical Exam Report from the current year
3. Health History Form
4. Dental Exam
5. Getting to Know Your Child
Authorization for Medications to be Taken During School Hours
Any child requiring medication during school hours must submit an authorization form completed by a doctor with the child’s name, medication name, dosage and time to be given. This is also a New Jersey state law and there can be no exception. At no time is a child of any age allowed to carry any medication on his/ her person. All medication, prescription or over the counter, must be dispensed through the nurse’s office.
Vision Tests for Students Entering School
Please be advised that we do not require a vision test, but I strongly recommend that your child have his/ her eyes examined by a qualified physician before the beginning of the school year. This is especially important for children whose parents wear glasses. Young children may not realize that their vision is abnormal and so they will very rarely complain. Students' vision will be tested on a yearly basis in the nurse's office.
